Choose Women's for standard-width shoes: dress shoes, slimmer sneakers, lower-volume footwear.
Choose Men's for wider or roomier shoes: work boots, athletic sneakers, or when you want more surface area under the heel.
Width is really about the shoe, not the wearer. Either width can work for anyone. Choose by the shoe you'll wear it in.
A conservative starting point: begin around half of the measured leg-length difference, then increase gradually as the body adapts. Most people settle within a millimeter or two of that first choice.
